If you are having difficulty doing depression screening with your ESRD patients, here are 3 websites you could use. The Multicultural Mental Health Resource Centre, the Minnesota Department of Health, and The National Council below have translated the PHQ-9 in several different languages. PHQ Depression screening is required by CMS so social workers should be using a depression screening tool. The GAD-7 screens for anxiety. Anxiety and depression are often seen simultaneously so I have attached both for your review. If the patient screens with high scores on either or both screening tools, then the attached Depression and Suicide Screen Flow Chart is a tool that can be used for possible next steps.
Safety Planning Intervention: can be used as a step-by-step guide for the patient interaction processes once the mental health need has been determined.
