What is a sprint?
An improvement sprint is a rapid implementation program designed to amplify the impact of your facility’s efforts around a given topic, such as readmissions/transitions in care. This improvement sprint is a collaboration between Midwest Kidney Network and our larger quality improvement organization, Superior Health Quality Alliance.
What makes this endeavor unique and worthwhile?
Because this improvement sprint is in collaboration with Superior Health, hospital staff are registered participants in this project. This provides a unique opportunity to work alongside our inpatient colleagues, bringing together the inpatient facilities and the outpatient ESRD facilities as we work together to reduce readmissions. This is a great opportunity to start opening communication pathways between inpatient and outpatient facilities, and to better understand each other’s barriers and opportunities in transitions of care.
